Chase Short Sale Documents

If you have a Home Mortgage with Chase and are considering a Short Sale (First or Second Mortgage – or Both) the following documents are required to be completed and sent to Chase to begin the process.

We are here to assist you every step of the way and will keep you up to date as we submit the short sale and work with the bank to complete it.

If you have received any notices from the bank and specifically a Notice of Default, we will need to see a copy of that as well as the documents listed below.

  1. The Short Sale Information Packet outlines the documents you will need to complete and the documents you will need to provide.
  2. Chase provides you with a check list of the Required Documentation.  It is a useful tool in organizing your paperwork.  Please be sure to provide us with each applicable document (specific to your circumstances).
  3. Authorization to Provide and Release Information (page 3 of 10) is critical to allow the lender to speak with us about your loan.  This document can be sent ahead of time, while you fill out and gather the rest of the information, since it takes several days to get this into the system.  Please complete and return this one to us immediately.
  4. Request for Consideration of Short Sale Form (pages 4 – 5 of 10) need to be filled in as completely and accurately as possible.  We are happy to assist you with this process!
  5. Request for Consideration of Short Sale Acknowledgment and Authorization and Signature page (pages 6-7 of 10) need to be read and then signed on page 7.
  6. Request for Transcript of Tax Return Form 4506T (pages 8-9 of 10) allow the bank to obtain summary copies of your tax return.  Even though we sent the complete returns, they will verify the information using this form.
  7. Dodd-Frank Certification – (page 10 of 10)  check the appropriate box and date and sign.
